• Completed notification of loss (förlustanmälan), on the back side of the white section of the passport application form.
• Copy of your census registration record, available from the local tax office where you were most recently registered.
• Driver's license or other valid photo ID.

As of May 1, 2012 the Embassy in Washington, DC will only issue emergency (temporary) passports by appointment on Mondays and Fridays between 10:00 am - 12:00 pm. To schedule an appointment please call the Embassy reception at (202) 467-2600.

New passports are normally issued within 1-2 weeks from the time of application. If you are traveling in the next few days and need a passport immediately, you may apply for a emergency (temporary) passport at any of the following consular locations:
