Lost or Missing Passports

• Completed notification of loss (förlustanmälan), on the back side of the white section of the passport application form.
  
  
• Copy of your population registration record, available from the local tax office where you were most recently registered.
  
  
• Driver's license or other valid photo ID.

New passports are normally issued within 1-2 weeks from the time of application.  If you are traveling in the next few days and need a passport immediately, you may apply for a emergency (temporary) passport at any of the following consular locations:

• Ft. Lauderdale
• Houston
• Las Vegas
• Los Angeles
• Minneapolis
• New York
• Raleigh
• San Diego
• San Francisco
• Tampa
• Washington DC

The fee for an emergency passport is $200.
