If the passport number is missing, you must specifiy if the missing passport was regular, extra, or provisional. This is to allow cancellation of the missing passport.

Lost or Missing Passports

If your passport is lost or missing, the following is required:

Completed notification of loss (förlustanmälan), on the back side of the white section of the passport application form.
Copy of your population registration record, available from the local tax office where you were most recently registered.
Driver's license or other valid photo ID.

New passports are normally issued within 2-3 weeks from the time of application. If you are traveling in the next few days and need a passport immediately, you may apply for a emergency (temporary) passport at any of the following consular locations:

New York
Boston
Chicago
Ft. Lauderdale
Los Angeles
Minneapolis
San Francisco
Washington DC

The fee for an emergency passport is $180.
